What is Personal Information and why do we collect it?

Personal information is information or an opinion that identifies an individual. Examples of personal information we collect include: names, addresses, email addresses, phone numbers, facsimile numbers, occupation, health information, date of birth, demographic information such as postcode, preferences and interests, other information relating to customer surveys, offers and competitions.

How we use your information

As health care providers we are required by law to keep certain information about clients. We collect your personal information for the primary purpose of providing our services and information to you and for marketing. We may also use your personal information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. You may unsubscribe from our mailing/marketing lists at any time by contacting us by email. When we collect personal information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

  • To maintain accurate internal record keeping. 
  • Your information may be used to offer you the service of other providers for supply of products and services, your permission will be requested before this occurs. You may opt out by emailing us. 
  • To contact you via phone, fax, email, or mail in relation to your health needs. 
  • An opt in database for offers and promotions may be used. You may opt out by emailing us.

Sensitive information relates to information of a more personal nature, such as but not limited to personal opinions, racial or ethnic origin, political opinions, religious beliefs, or health information.

This type of information is kept strictly confidential and stored securely. Only myNaturopath practitioners you are disclosing the information to have access to this information.

In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Disclosure of Personal Information

To assist us in providing Naturopathic and Counselling services, we may disclose this information to third parties with your consent.

  • other health care providers
  • pathology laboratories
  • our client management software provider
  • our accountant or solicitor
  • our supplement suppliers and distributors

We will also disclose your information if required by law to do so or in circumstances permitted by the Privacy Act – for example, where we have reasonable grounds to suspect that unlawful activity, or misconduct of a serious nature, that relates to our functions or activities has been, is being or may be engaged in, and in response to a subpoena, discovery request or a court order.

We may also disclose your information, on an anonymous basis, for education, demonstration or research purposes.

myNaturopath Clinic will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your personal information.

In order to protect your personal information we may require identification from you before releasing the requested information.

Maintaining the Quality of your Personal Information

It is an important to us that your personal information is up to date. We  will  take reasonable steps to make sure that your personal information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.
